6/6/2023 EnchantaVaughn Approved Behavior: At the start of the session, Anthony expressed that he needed time to gather himself so that he wouldnât take waste his therapy hours being mad and upset. After mom discussed a few things regarding Anthonyâs progress of behaviors Anthony finally gave his undivided attention in the session. Behavior: Anthony has verbalized that he feels like an outcast some days and no one can understand why he has so much hostility built up inside. Anthony stated that ever since moving back to live with his mom, nothing has been the same. Intervention: QMHP monitored individualized behaviors during point of contact. QMHP encouraged Anthony to listen to his mom and respect her decisions. QMHP encouraged Anthony to be the example for his siblings. QMHP encouraged Anthony not to follow others he sees that are doing wrong. QMHP challenged Anthony to trust himself and explore things he has experienced recently. QMHP reminded Anthony of his home goals in compared to other goals so that he knows the difference between personal and professional objectives. QMHP intervened to assist in explaining behaviors to Anthony and what he needs to work on if he wants to meet his goals and objectives more consistently. Intervention: QMHP will provide strategies for coping and self-control when faced with family conflict and peer confrontations. QMHP reviewed and gave feedback regarding ADHD and its causes and effect to well-being and overall performance in the home. QMHP isolated Anthony from distractions to decrease disruptive behaviors and to reduce episodes. QMHP assisted Anthony in normalizing the changes that are occurring around him socially and verbally. QMHP implemented a routine that will encourage Anthony to be responsible and promote self-confidence daily. Response: Anthony anger has been pretty intense as described lashing out at his mom and siblings for no reason. Anthony has overreacted with anger to minor frustrations and irritants but was able to reduce self-defeating thoughts after one on one with QMHP. Anthony is working on walking away and telling an adult before responding to anger provoking situations. Anthony understands that problem solving starts with thinking before acting. Anthony need improvement with expressing himself appropriately around family and peers. Response: Anthony knows what is expected of him and is trying to make better choices moving forward. Anthony verbalizes positive feelings about himself but recognizes areas he needs to work on as well. Anthony admits when he is wrong and apologizes for it. Anthony is learning to calm down, think first, and respond without anger and frustration. Anthony reduces sadness and finds more excitement in learning about his mental illness during todayâs in home visit. Anthony was happy he was able to cooperate and finish out the session.
